- Primary contact person and contact information
- Valid FIFA Agent License (If work with minor in Canada, please produce a valid Criminal Check or Police report).
Note: only individuals with valid FIFA agent License will register as full member. Those who intent to get trained to become agent will be register as aspiring Agent.
